2025 Volunteer of the Year Nomination Form

This award recognizes one individual who has made an outstanding contribution to the Tri-Lakes community, making it a better place to live and work through their volunteer service. 

Criteria:  
Nominee must have demonstrated outstanding leadership in service to the community (including serving during 2025 with at least one organization that is a Chamber member). Emphasis should be placed on personal achievements including leadership, organization, success, and spirit that embodied a volunteer effort in 2025.

Nominators: 
Please notify the person you nominate and provide them with the date, time, and registration information so they can plan to attend and bring co-workers and family. Self-nominations are encouraged.

Awards will be presented at the Tri-Lakes Chamber of Commerce Annual Dinner and Silent Auction,
Thursday, April 2, 2026, at Phil Long Music Hall at Bourbon Brothers.

Nominations are judged by committee solely on the information provided on this application. 
Additional sheets may be attached as needed.

2025 Board Members are not eligible. 

Nomination forms are due prior to 5 p.m. on Wednesday, March 25, 2025.
